Dr. DiGiacomo can also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea as well.
Once the exam is complete, Dr. DiGiacomo will complete Form I-693 and seal it in an envelope for you to submit directly to USCIS along with your other immigration documents. It is important that you obtain a copy of this form for your personal records before sending it off, as USCIS will only accept a completed version that has not been opened or altered in any way.
